Creating a blood glucose (blood sugar) check order and creating an insulin medication order

FAQ: I need to add blood sugar readings with a glucometer into MAR, I need to be able to track the results every time. Also need to track the insulin injection site

Part 1: Creating a Custom Blood Glucose Check

1. Navigate to the EMR Module

  • Go to EMR > Medication List.

  • Click Add Medication.

2. Add a Custom Medication for Blood Glucose Check

  • If integrated with a pharmacy, you may see an entry like True Plus 33G Lancets — you can use this.

  • If not, manually create a custom entry:

    1. Click Add Medication.

    2. Enter Drug Name: Check blood glucose.

    3. Set Drug Route: Custom or Not Applicable.

    4. Set Dose: 1.

    5. Set Strength: Check.

      • Reason for This: Allows you to define an action instead of a measurable dosage.

    6. Set Drug Form: Custom.

    7. Add Instructions: Write clear instructions like Check blood glucose using fingerstick before breakfast.

      • Reason for This: The system's auto-generated text like "Take one custom QD one time daily custom" is nonsensical and must be customized for clarity.

    8. Set Reason for Use: Diabetes or Glucose check.

3. Set Frequency and Health Monitoring

  • Set Frequency: e.g., One time daily (match the provider's order).

  • Scroll down to Restrictions.

    • Check Health Monitoring.

    • Select Blood Glucose.

    • Do not check “Hold order” — this entry is for monitoring only.

4. Save and Verify in MedPass

  • After saving, go to MedPass.

  • You’ll see the Check Blood Glucose entry.

  • Select the Blood Glucose vital.

  • Enter a value and click Save.

  • Click Given, then Save again.

  • Optionally, enter location details (e.g., Left hand).


Part 2: Adding an Insulin Order

1. Navigate to Medication List and Search for Insulin

  • In EMR > Medication List, click Add Medication.

  • Search for and select Insulin.

2. Complete Medication Details

  1. Drug Route: Subcutaneous.

  2. Strength: 100 units/mL.

  3. Drug Form: Injection, solution.

  4. Frequency: One time daily.

  5. Start Date: Select Today.

  6. Time of Administration: e.g., 10:00 AM.

  7. Dose: e.g., 0.20 mL for 20 units.

  8. Reason for Use: Diabetes.

3. Configure Health Monitoring and Hold Conditions

  • Under Restrictions, enable Health Monitoring.

  • Select Blood Glucose.

  • Configure hold conditions (e.g., Hold if glucose <100 mg/dL).

4. Save and Document Administration

  • Save the order.

  • Go to MedPass.

  • Record current blood glucose level if needed.

  • If values justify insulin, click Given.

  • Enter injection site (e.g., Left abdomen around 40 area).


Notes and Best Practices

  • Instructions Field: Always revise the default text. The autogenerated instructions often do not make clinical sense and must be rewritten for accuracy.

  • Health Monitoring Link: Tying a medication or order to a health monitoring vital ensures the value is recorded at administration, improving documentation and oversight.

  • Site Documentation: Entering where the injection or fingerstick was performed improves tracking and complies with nursing best practices.
