Adding a Sliding Scale to a Pharmacy Insulin Order

How to approve an insulin order from the pharmacy and then edit it to add a sliding scale

FAQ: I have a pharamcy order that has a sliding scale, how do I add that to the order?

  1. Approve the Pharmacy Order

    • Navigate to the Pharmacy tab. If the order is not visible, check under Outdated Orders.

    • Click on the insulin order, then click Approve. Proceed past any warning messages.

    • Click Add Order and proceed past the accuracy duplicate check.

  2. Create the Initial Medication Order

    • On the medication form, ensure the Start Date and administration times are correct.

    • Under Drug Classification, do not select "Sliding Scale" yet.

    • Scroll down and select the blood glucose vital.

    • Click Create this medication.

  3. Navigate to Edit the Medication

    • Go to the resident's medication list.

    • Find the newly added insulin order and click the three dots icon, then select Edit.

  4. Add the Sliding Scale

    • In the edit screen, copy the sliding scale instructions from the original pharmacy order.

    • Change the Drug Classification to Sliding Scale. This will reveal the sliding scale table.

    • Select Body Area to enable injection site tracking.

    • Paste or manually enter the dosage rules from the instructions into the sliding scale table. For example:

      • Less than 70, give 0 units.

      • Between 71 and 200, give 12 units.

      • Between 201 and 300, give 14 units.

    • Add any specific instructions, such as "call MD," in the corresponding fields.

  5. Finalize the Order

    • Once the table is complete, you can remove the copied text from the main instructions box, as the sliding scale will now automatically populate this information.

    • Set any hold parameters, such as holding the medication if the blood glucose is less than 70.

    • Check the box for "I'm aware of the changes."

    • Click Save.

  6. Verify on the Medpass

    • On the Medpass, when you select the insulin, you will be prompted to enter the resident's blood glucose level.

    • The system will automatically calculate the correct dose based on the sliding scale.

    • Click Give and mark the injection site on the body silhouette to complete the administration.
