⓷ Create Your First Resident Profile

After creating the facility profile, start by adding a resident profile.

FAQ: What do I do after I create my facility? I need to add my first Resident.

  1. Access the Resident Creation Tool

    • After creating your first facility, click on the facility image to view its details.

    • Click the Synkwise logo or navigation bar, then select the icon of a person with a plus sign to start adding a resident.

  2. Enter General Information

    • Fill in first name and last name (must match pharmacy records if connected).

    • Enter the date of birth either by selecting from the calendar or typing it in.

    • Provide the date of admission, gender (optional), and room number.

    • Select the facility if you manage more than one.

    • You can also assign staff and later add an “About Me” summary in third person.

  3. Add Allergies and Diagnoses

    • Enter any known allergies (e.g. peanut allergy).

    • For diagnoses, start typing or paste the ICD-10 code or description. You can use codes from hospital discharge papers or type them manually.

  4. Connect Pharmacy Information

    • Choose a pharmacy from your state list.

    • The selected pharmacy’s contact info (phone/fax) will auto-fill in the resident’s contact section.

  5. Create the Resident Profile

    • After reviewing all info, click Create.

    • This saves the resident and takes you to their full profile view.

  6. Update Profile with Additional Information

    • Click on the resident profile to update or complete more sections.

    • Add a photo by dragging/uploading.

    • Assign labels to categorize or tag residents (create new ones if needed).

  7. Complete Background and Health Info

    • Fill in fields such as height, weight, religion, language, code status (e.g. DNR), vaccination records, dietary needs, insurance details, and funeral preferences.

    • Upload important documents like the POLST/DNR form to appear in emergency reports.

  8. Save Updates

    • Once all info is entered, click Update at the top of the profile page.

    • Use the Help button in the bottom-right corner of Synkwise for assistance.
