FAQ: I need to create an incident for a resident who fell in their room.

Access the Resident’s Profile
- Go to the resident's profile in the system.
- Click on the "Reports" button to create an incident report.

Navigate to the Incident Reports Section
- The system will direct you to the "Resident Reports" section.
- Select the "Incidents" subsection.
- Use the "Filters" button to sort incidents by status (draft, completed, finalized) or by type (falls, injuries, medication errors, etc.).

Create a New Incident Report
- Click on "Create Incident Report."
- Select the type of incident report if multiple options are available.

Enter Incident Details
- Fill in the required fields:
- Reported by
- Location of the incident (e.g., bedroom)
- Date and time of the incident
- Type of incident (e.g., fall)
- Provide a detailed description of what happened.

Describe the Resident’s Condition
- Document the resident’s physical state (e.g., bruises on shoulders, elbow, or hands).
- Upload images if necessary.

Determine the Cause
- Review any medications taken in the last 12 hours.
- Check if vitals were recorded in the past hour.
- Identify the possible cause of the incident (e.g., accidental fall).
- Document whether emergency services were called.
- Record the resident’s statement about the incident.

Complete the Investigation
- Add the incident to the facility’s log if necessary.
- Document the outcome (e.g., resident is okay).
- Record witness statements and investigative findings.
- Determine if the care plan needs updating based on the incident.

Notify Relevant Parties
- Document whom you notified (e.g., yourself, nurse, family member).
- Include the date, phone number, and other relevant details.
- Note if authorities or state investigators were contacted.

Save and Finalize the Report
- Save the report as a draft if more edits are needed.
- Click the pencil icon to make changes.
- Finalize the report by clicking on the three-dot menu and selecting "Finalize."

Print, Fax, or Email the Report
- After finalizing, go to the three-dot menu.
- Choose to print, send via fax, or email the report.
