Add a date and time, who, and the reason why a medication was discontinued under the medication list
FAQ: How do I discontinue a medication or put an end date to stop the order?

Access the Medication List
- Navigate to the EMAR section in Synkwise.
- Locate the medication you want to discontinue.

Click on the Action Button
- Find the three-dot action button next to the medication.
- Click on it and select "Discontinue."

Verify Past Administration in the Medlog
- Check the Medlog to confirm when the medication was last administered.
- Ensure that the medication is not being discontinued after it has already been given.

Set the Discontinuation Time
- By default, the system selects the current time when you click "Discontinue."
- If needed, manually change the time to a specific point (e.g., 10 a.m.).

Enter Discontinuation Details
- Select the provider who discontinued the medication.
- Enter the reason for discontinuation.
- Optionally, attach a PDF or other documentation.

Confirm and Discontinue
- Click "Discontinue" to finalize the process.
- The medication will now appear under the "Discontinued" filter in the medication list.

Restoring a Discontinued Medication (if needed)
- Click on the "Discontinued" filter to find the medication.
- Click the three-dot action button and select "Restore."
- Type "Restore" to confirm, and the medication will return to "Active" status.

Alternative Method: Setting an End Date
- Edit the medication and set an end date (e.g., January 29th at 8 a.m.).
- Save the changes, and the medication will be marked as discontinued after the set date.
- The Medlog will retain previous signatures even after discontinuation.
