Transferring Data from an Outdated Care Plan Template to the New Care Plan Template

Transfer data from an old care plan version to a new care plan after updates have been made

FAQ: I'm trying to create a new care plan, and none of the information from the old care plan carried over. Where is my data?

1. Open the Resident’s Latest System Default 1 Care Plan

  • Navigate to the resident profile.

  • In Care Plans, click View next to the plan labeled Care Plan — System Default 1.

2. Finalize the Old Care Plan (If Not Already Finalized)

  • Inside the open plan, click Finalize.

  • Confirm the action.

3. Create a New Care Plan and Pre-Fill Data

  • Back on the resident’s Care Plans list, click New Care Plan.

  • In the creation dialog, turn on Pre-Fill Data from Other Source.

  • From the drop-down, select Care Plan — System Default 1.

  • Click Create.

4. Review and Finalize the New Negotiated Care Plan

  • Confirm that all expected sections are populated.

  • Complete any new required fields added in the Negotiated template.

  • Click Finalize when ready.

Important Notes & Limitations

  • Works only on in-app care plan instances. If the original plan exists only as an uploaded PDF, the pre-fill option will not detect it.

  • Sensitive resident data is transferred exactly as stored; redact or update information as needed before finalizing.

  • You can repeat this process whenever further template updates occur.
