Upload your completed and signed service plans to stay organized and compliant

FAQ: How do I add a signed service plan?

  1. Navigate to the Care Plans Page

    • Open Synkwise and go to the Care Plans section.
    • You will see a list of previously uploaded care plans.
    • A folder with a clipboard icon means a care plan was uploaded as a PDF.
    • A clipboard with a pencil icon indicates a care plan created using the Form Builder.
  2. Delete an Existing Care Plan (If Needed)

    • If you need to replace an existing care plan, delete the previous version before uploading a new one.
  3. Upload a New Care Plan

    • Click on the Upload button.
    • Select Upload Document and browse your computer for the signed care plan (PDF format).
  4. Select Care Plan Type and Date

    • Choose the appropriate care plan type (e.g., Routine Care Plan).
    • Enter the date the care plan was completed.
    • Set a renewal reminder (e.g., six months or one year from the completion date).
  5. Finalize the Upload

    • Ensure the care plan is 100% complete before uploading.
    • Click Upload and wait for the system to generate a new care plan number.
  6. View and Manage the Uploaded Care Plan

    • Click View to see the care plan in PDF format.
    • You can download, email, or send it for signature.
